Stay safe and healthy abroad
We offer this service to Rivermead Gate Medical Centre patients only, this is not a service that is open to anyone.
If you are travelling abroad you may need to be vaccinated. Please complete an online travel questionnaire (one form must be completed for each person travelling). This is to allow the nurse time to prepare for the appointment. Forms can be completed online, downloaded and emailed in or collected from reception.
Once you have submitted your forms, please arrange a 10 minute travel vaccination telephone consultation with the nurse to discuss your needs.
It is important to make this initial appointment at least six to eight weeks before you travel. The latest date we can receive forms is three weeks before departure. After that date, you will be asked to attend a travel clinic for advice.
We currently offer the following NHS Vaccines : Tetanus, Polio, Diphtheria, Typhoid, Cholera and Hepatitis A.
More complex travel vaccines can be obtained from a travel clinic.
