Before you complete the online forms please use this link to check if your current student address is within our practice boundary.
We are asking that where possible all new patient registrations use the online registration form. This will then be sent directly to our registration clerk for processing.
Please allow 14 days for your registration to be processed, please note we are no longer accepting registrations at our branch site as all patients will be treated at our main site of Rivermead Gate Medical Centre.
It is important that you complete all of the questions on the online registration form your application may be rejected or delayed.
An up to date contact number and email address for you must be on the form in order for us to process your paperwork.
If you do not have access to a computer or smart phone, registrations forms can be collected from the reception desk, however this should be only be in exceptional cases.
Important information
Please note that whilst you are living in our catchment area, you are registered with Rivermead Gate Medical Centre. Should you return home, visit friends or relatives in the holiday we are no longer responsible for your care. Whilst staying out of the Practice area you will need to seek medical attention from your local GP Practice or contact NHS 111. This would be for routine medical problems and medication or advice.
In the event that you require urgent medical advice please contact the A&E Centre close to where you are staying or contact NHS 111 for advice.
