To register with the practice during COVID-19:
Download the and complete the registration form and new patient questionnaire. Scan or photograph the documents with photo ID and email them to [email protected]
Ensure we have your contact details as we may need to call you for additional information.
Please allow 7 days for your registration to be processed.
We are no longer accepting registrations at our branch site at Wood Street.
All new patients applying to register at the branch site will be registered and treated at Rivermead Gate Medical Centre, 123 Rectory Lane, Chelmsford, CM1 1TR.
Should you wish to proceed with your application, you should understand that this will be on the basis that your health services will be provided at Rivermead Gate Medical Centre and you will need to attend there for all medical treatment. We will not be able to offer you services at the branch site.
If you are taking regular medication please book a telephone appointment with the Nurse Practitioner or GP.

If you are registering children under 16 years send their immunisation history with the forms so that we can update our records.
You will have a named accountable GP who is responsible for your overall care. However, you can still see your GP of choice. Contact the practice if you wish to know who your named GP is.
