We are asking that where possible all patients use the online registration form for each member of the household, this will then be sent directly to our registration clerk for processing.
Please allow 7 days for your registration to be processed, please note we are no longer accepting registrations at our branch site as all patients will be treated at our main site of Rivermead Gate Medical Centre.
Should you wish to proceed with your application, you should understand that this will be on the basis that your health services will be provided at Rivermead Gate Medical Centre and you will need to attend there for all medical treatment. We will not be able to offer you services at the branch site.
If you are taking regular medication please book a telephone appointment with the Nurse Practitioner or GP once you are registered.
Temporary patients
Any person in the UK staying for more than 3 months can be registered as a permanent patient. Complete an online registration form or download the forms below.
A person staying up to 3 months can only registered as a temporary patient. Complete an online temporary patient registration form.
Child registration
If you are registering children under 16 years send their immunisation history with the forms so that we can update our records.
Care home residents

Student registration
If you are a student wanting to register with this practice please go to our Student Registration Pack for further information and online forms.
Asylum Seeker registration
If you are an asylum seeker wanting to register at this practice, please use our Asylum Seeker registration online form.
