If you would like a resident to be registered with Rivermead Gate Medical Centre, please complete the registration details below and return the forms to the care home team or the practice reception.
Once we receive the completed information, we will process the registration and update our records. If you require any assistance completing the forms, please speak to a member of the care home staff or contact the practice directly.
Please ensure the following information is provided on the registration form:
- Full name
- Date of birth
- NHS number (if known)
- Current address and care home details
- Previous GP practice details
- Next of kin/emergency contact information
- Current medication information (if applicable)

Dementia Intensive Support Service (DISS)
The Dementia Intensive Support Service (DISS) provides joined-up mental health and physical health support for people living with dementia or suspected dementia in mid Essex. This service offers: rapid support for people who are in crisis; intensive support and treatment at home; advice around dementia diagnosis; signposting to further advice and support; help in accessing Social Care support. DNACPR, Peace Plans and RESPECT forms are available to download from our website. These documents support advance care planning and help ensure that residents’ wishes are understood and respected.
DNACPR (Do Not Attempt Cardiopulmonary Resuscitation)
A DNACPR form records a clinical decision that cardiopulmonary resuscitation (CPR) should not be attempted if a person’s heart or breathing stops. It only relates to CPR and does not affect any other treatment, care, comfort measures, or clinical decisions.

Peace Plan
A Peace Plan helps residents, families, care home staff, and healthcare professionals plan future care and support. It records the individual’s wishes, preferences, and goals of care, helping everyone involved to make informed decisions, particularly when a person’s health deteriorates.

RESPECT form
The Recommended Summary Plan for Emergency Care and Treatment (RESPECT) form summarises a person’s preferences for future care and emergency treatment. It helps healthcare professionals understand what matters most to the individual and provides guidance when urgent decisions need to be made, especially if the resident is unable to communicate their wishes.

Vaccination consent form
The Vaccination Consent Form enables residents, or their legal representative where appropriate, to provide consent for recommended vaccinations such as seasonal influenza, COVID-19, pneumococcal and other routine immunisations. Having consent documented in advance helps ensure residents can access vaccinations promptly and safely when clinics are arranged

Care home staff are encouraged to discuss these documents with residents and their families where appropriate and arrange a conversation with a GP or clinician to complete or review them. Having these plans in place helps ensure that care is provided in accordance with the resident’s wishes and best interests.

Weekly Care Home ward rounds
Our dedicated care home team conducts weekly ward rounds to review residents’ health needs, medication, ongoing care plans and any clinical concerns raised by care home staff. To make the most of these visits, please ensure any issues requiring review are communicated to the practice in advance. An up to date list of residents requiring
Practice input should be submitted prior to the Ward Round so that the clinician can plan their time.
Complete our online Ward Round Proforma – to be submitted the evening before the ward round or by 9am on the day.
Palliative Ward Rounds
Palliative ward rounds help plan anticipatory care for a residents last months of life, deterioration in their health and ensure their wishes are upheld wherever possible.
Discussion will be held around their preferred place for end-of-life care, reducing unnecessary hospitalisation and improving anticipatory.advance care planning to ensure that residents live well up until the end of their life.
Peace plans, DNACPRS, Respect forms can all be completed by the clinician carrying out the visit and additional support requested if needed.
These are held monthly at our Care Home and three monthly at our Nursing Home.
