Before you complete the online forms please use this link to check if your care home is within our practice boundary.
To register at the practice:
We are asking that where possible all new patient registrations use the online registration form for each care home resident this will then be sent directly to our registration clerk for processing.
Please allow 7 days for your registration to be processed, please note we are no longer accepting registrations at our branch site as all patients will be treated at our main site of Rivermead Gate Medical Centre.
EOL planning
Once you have completed the online registration form to register at this practice, please could you also download and complete a DNACPR form.

Temporary care home residents
For any temporary care home residents, please complete the GMS3 form online.
Alternatively, you can download and complete the GMS3 form. This will then need to be emailed to rivermead.registrations@nhs.net
