Support for People living with Dementia and their carers

Dementia Adventure are currently running a pilot in Essex for a new service offering Supported Days Out for people living dementia and their carers to boost their wellbeing by getting outdoors.

We would like to raise awareness of this service which we feel will help those that are feeling anxious since lockdown about getting back outdoors.

The aim is to offer a supported outing to build confidence and enable people to get outdoors and connect with nature regularly themselves.

Transport to the venue is not included as the focus is on enabling people to replicate a similar activity under their own steam, however they can support people to find local community transport.

DISS:

Dementia Intensive Support Service (DISS) provides joined-up mental health and physical health support for people living with dementia or suspected dementia in mid Essex.

First: functional intensive response and support team

The FIRST team provide intensive mental health support for older adults. We can provide interventions in your home instead of a hospital admission, with your agreement and if it is safe to do so. If you are to be admitted to hospital, the FIRST team will work closely with you, the hospital team and other agencies to plan your discharge and continue your treatment at home.

We accept referrals from health professionals who will have carried out an assessment of your needs and risks. We work alongside services you may already be getting support from.
