Please follow this link to the Gov.uk website to make an application for a Blue Badge
Your application can be made online, however you will need to upload documents as supporting evidence and a current passport photo. There is also a charge of £10 for the application.
Initially the application must be completed by the patient, however further down the line the Local Council may request a Dr’s supporting letter.
If this is the case, please request this via our [email protected] email account but be aware that this is considered non-NHS work and will therefore incur a charge.
